Some twenty years ago, I developed a pain in the lower right quadrant of my abdomen that seemed to travel up and to the side radiating into my back on the right side. The pain would come and go, accompanied by minor abdominal bloating constipation and or diarrhea. My doctor of that time was what you might consider, an old fashioned, tell it like it is country doctor, who though it might be a gallbladder issue. Over the course of several months several tests were performed including but not limited to an ultra sound, CT scan and a high fat intake study, in an attempt to force the gallbladder to react negatively. Type 2 Diabetes Mellitus (T2DM) is an epidemic and alarming metabolic disorder. The Centers for Disease Control and Prevention (CDC) estimated that in 2005 the prevalence of diabetes in the United States was 20.8 million. Diabetes is diagnosed in 14.6 million persons and undiagnosed in 6.2 million. T2DM is 90% to 95% of prevalent diabetes. About 41 million people in the United States are believed to have pre-diabetes.
The main etiologic pathways of T2DM are insulin resistance and lack of compensatory insulin secretion. Insulin resistance is usually longstanding and begins at a young age because of heredity combined with environmental factors (sedentary lifestyle and calorie overconsumption leading to overweight). Longstanding insulin resistance is associated with dyslipidemia, central obesity, hypertension, and hyperglycemia. Thus, it is very frequent the coexistence of cardiovascular disease and T2DM.

Inflammatory bowel disease (IBD) describes the spectrum of chronic intestinal inflammation from Crohn's disease (CD) to ulcerative colitis (UC). Although this term is very misleading (because there are much more diseases which also imply intestinal inflammation, such as infective diarrhea for example) it is universally used as synonym of the CD/UC pair.
IBD seems to occur as a consequence of a persistent and inappropriate immunologic response to gut luminal antigens. The absence of enteric parasites, in developed countries, and defects in mucosal innate defenses are recent additions to the many hypotheses on the pathogenesis, which remains very doubtful and unknown. However, independently from the cause, CD and UC usually respond to a similar range of anti-inflammatory and immuno-modulator therapy to induce and maintain the remission stage.
